The Madison County Health Department staff is providing Influenza Immunization Clinics for state employees/retirees BY APPOINTMENT ONLY. Health Department nurses will administer the flu shots on Thursday, October 5, 2023 from 9:00 a.m. to 12:00 p.m. and Thursday, October 19, 2023 from 1:00 p.m. to 4:00 p.m. in the Cafeteria at the ISP/IDOT building on Eastport Plaza Drive in Collinsville.

All individuals getting flu shots this year MUST BRING THEIR DRIVER’S LICENSE OR A VALID ID PLUS THEIR INSURANCE CARDS (NO EXCEPTIONS).
● Teamsters employees will NOT incur out of pocket expenses. You must bring your photo ID and Teamster’s Insurance Card.
● State employees will NOT incur out of pocket expenses. You must bring your photo ID and State Insurance card.
● Those with Medicare coverage will NOT incur out of pocket expenses. You must bring your Medicare card with you.
The Health Department has implemented an electronic system, therefore no questionnaires are necessary. Please share this important information with all employees and retirees.
